The documentation provided must be completed by an individual qualified to make the diagnosis stated in the application. The diagnostician should be an impartial individual who is not a family member of the student. In addition to submitting a completed Application, the following items should be included in all documentation submitted for consideration:  A clear statement of the disability.  A summary of assessment procedures and evaluation instruments used to make the diagnosis.  Medical information relating your need, including the impact of medication on your ability to meet the demands of the college environment.  Information on how the impairment impacts your ability to participate in the activities at Calvary.  Suggestions of reasonable accommodation which might be appropriate at the college level. Recommendations should be supported by the diagnosis.  Information on how the impairment substantially limits one or more major life activities as defined by Section 504 and the ADA.Documentation Required for ADHD
