Student Self Report- In determining eligibility, the student is the most important source of information to gain an understanding of how one's impairment limits academic performance. A student's explanation of his/her disability is an important tool in the accessibility process and may sometimes, but not always, be sufficient for registration with the Disability Services office. Observation and Interaction- The interview process allows our disability professionals to validate student self-reports as we observe their language, performance and strategies during the intake session. Information from Third Parties- Documentation from external sources may consist of educational and/or medical records as well as assessments and reports from health care professionals, school psychologists, and other educational providers. The relevance and value of this documentation will be based on the credentials of the provider, level of detail in the report, the context in which it was written, and comprehensiveness of the narrative.
